Coos Bay Schools is partnering with Waterfall Community Health Center to offer free and convenient flu vaccinations for all interested students and staff.
Clinic Schedule
All clinics will run from 12:30 PM to 4:00 PM:

Tuesday, October 27: Eastside & Millicoma (Location: Eastside Conference Room)

Wednesday, October 28: MHS & MJH (Location: Heritage Hall)

Thursday, October 29: Madison & Sunset (Location: Madison Stage)
How to Register

Consent Forms: Signed consent and registration forms are required before receiving a vaccine. You can pick up a consent form at any school office, or click the button below.

Staff Note: School staff participating must bring a copy of their insurance card.

Age Requirement: A parent or legal guardian must sign the consent form if the student is under 15 years old.
Honoring Blake
We hold this clinic annually in honor of Blake Crane, a Coos Bay Schools student (Class of 2021) who tragically passed away from flu complications in 2020 at the age of 17. We share his story to help raise awareness about the serious dangers of the flu and to keep our school community protected.
